Benevolence Request

Benevolence Protocols

This form is used to request benevolence (supplemental taxable income). If you receive a total of $600 or more for the current tax year, a 1099 IRS Tax form will be given to you at the end of the year.

How to complete the form:

Note: If a document does not follow these protocols, it will be returned to you.

Physical Form Instructions:
  1. The form needs to be copied to your personal Google Drive and all four tabs of the form need to be filled out accurately and then printed on 8 ½” by 11” pages (full page view, portrait). If not, it will be returned to you. If it is missing pages, the form will be returned to you as incomplete. The form must be completely typed, no handwriting except for a signature in black or blue ink.
  2. Attach any accompanying receipt(s) and supporting documents to the back of the form.
    1. All of the receipts need to be taped to a blank page that is stapled to the form (add more pages if necessary) or put in an envelope and stapled to the form.
  3. This form will not be accepted without a social security number.
  4. **Need to have the requestor’s signature.**
Electronic Submission Instructions:
  1. All four tabs of the form need to be filled out accurately (including your signature, scanned and pasted into the appropriate space) and then saved as a PDF (or printed to a PDF, same thing) as a 8 ½” by 11” page document (full page view, portrait).
  2. All of the receipts need to be scanned and/or saved as a PDF and merged to the initial document. So you only submit one file.
  3. This form will not be accepted without a social security number.
  4. **Need to have the requestor’s signature.**

Approval process:

  1. Request needs to come to the House Church Leader
  2. House Church Leader determines if the request is valid, then requests approval from the shepherds.

Admin process:

  1. Need to have ministry approval’s signature: only Drs. Ken & Cheryl Chin, Damon & Vicki James, or Ron Harding can sign.
    • If one of the signatures is missing, the form will be returned to you as incomplete.
  2. To receive a check or PayPal amount on Tuesday, the form needs to be submitted by midnight on Sunday to dc.administrators@usd21.org.
    • If a physical form is submitted on Tuesday, it will take one-week turn-around time for the check to be processed.
